We are seeking a skilled Registered Nurse to join our team as a Discharge Lounge Specialist at HCA Florida North Florida Hospital. As a key member of our healthcare team, you will play a vital role in ensuring a seamless and efficient transition for patients coming in and out of acute care hospital settings.
Key Responsibilities- Conduct thorough nursing assessments to identify patient needs and develop individualized care plans.
- Collaborate with interdisciplinary teams to ensure timely and effective discharge planning.
- Provide education and support to patients and their families on post-hospital care and follow-up measures.
- Coordinate with healthcare providers to ensure accurate and complete discharge documentation.
- Ensure patients receive necessary resources and referrals for ongoing care.
To be successful in this role, you will need:
- Current RN licensure in the state of Florida or compact licensure.
- 1 year of experience in a healthcare setting, preferably in discharge planning or patient care coordination.
- Excellent communication and interpersonal skills.
- Ability to work in a fast-paced environment and prioritize tasks effectively.
