We are seeking a skilled Licensed Practical Nurse to support our residential programs and serve as a medical liaison for our IDT team. The successful candidate will be responsible for updating written plans for each individual, maintaining medical records, and participating in planning and implementation of training for DDI personnel.
Key Responsibilities- Participate in IDT conferences as required
- Develop written plans for each person with RN Supervisor and modify as necessary
- Maintain accurate and up-to-date medical records for each individual
- Participate in planning and implementation of training for DDI personnel
- Accompany individuals to medical appointments as assigned
- Ensure proper medical follow-up through on-call system
- Administer and store medications as necessary
- Attend monthly Nursing Meetings
- Complete incident reporting paperwork as required
- Ensure compliance with OPWDD regulatory standards
- Liaise with family members regarding medical issues
- Prepare monthly Medication Administration Records
- Provide 24-hour on-call support for medical emergencies with rotating weekend coverage
- N.Y. State Licensed Practical Nurse License required
- N.Y. State Drivers License preferred
- Knowledge of OPWDD Regulations preferred
- Experience with ID/DD preferred
