About the Role

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Case Manager. As a key member of our home health care team, you will play a vital role in delivering high-quality patient care and ensuring the best possible outcomes for our patients.

Responsibilities
  • Perform home visits and coordinate client care to ensure seamless delivery of services
  • Oversee the frequency of visits for each episode and communicate with the physician and office staff as needed
  • Collect and analyze accurate OASIS data to inform care decisions
  • Ensure medication profiles are up-to-date and lab values are reported to the physician in a timely manner
  • Attend weekly case conferences and monthly case manager meetings to stay informed and share best practices
Requirements
  • Current licensure as a Registered Nurse (RN) in the state of employment
  • Minimum of one year of clinical experience preferred
  • Demonstrate knowledge and skill in current nursing practice
  • Valid state driver's license and automobile liability insurance
