Job Summary

We are seeking a skilled Registered Nurse (RN) to join our team as a Case Manager. In this role, you will be responsible for coordinating patient care and overseeing the frequency of visits for our clients.

Key Responsibilities
  • Perform home visits and complete coordination of client care
  • Oversee the frequency of visits for the episode
  • Consult with physicians and office staff as needed
  • Accurately collect OASIS data and maintain current medication profiles
  • Ensure timely reporting of lab values to physicians
  • Attend weekly case conferences and monthly case manager meetings
Requirements
  • Current licensure as a Registered Nurse (RN) in the state of employment
  • Minimum of one year of clinical experience preferred
  • Demonstrate knowledge and skill in current nursing practice
  • Valid state driver's license and automobile liability insurance
