About the Role:

We are seeking a skilled Care Coordinator to join our team and coordinate care and services among various physical, behavioral, dental, developmental, and social service delivery systems. The successful candidate will be responsible for ensuring individuals receive the right care at the right time and becoming able to live successfully in their communities.

Key Responsibilities:
  • Maintain a caseload of Managed Care Plan (MCP) Members
  • Serve as Enhanced Care Management (ECM) Point of Contact/ Lead Care Manager for the MCP Members
  • Work collaboratively with treatment teams to provide comprehensive care
  • Oversee the provision of ECM services
  • Engage in outreach activities to connect eligible MCP Members with necessary resources

We are looking for an individual with a strong background in mental health services and experience working with diverse populations. A typical way to obtain the required knowledge, skills, and abilities is through graduation from high school or an accredited GED program and less than 2 years of experience as a provider of mental health services. Possession of a California driver's license and reliable means of transportation are also essential requirements.
