Care Coordinator RN

As a key member of our Elder at Home care team, the RN Care Manager plays a vital role in providing comprehensive care coordination services to our most medically fragile patients. This position requires a strong clinical background, excellent communication skills, and a passion for delivering patient-centered care in the home/community setting.
We are seeking a highly skilled and compassionate RN to join our team, providing care to patients in their homes with the goal of reducing ED utilization and hospitalization. This per diem position offers many opportunities for shifts, with a focus on providing urgent visits and assessments in the home with our provider group of eight physicians.
The ideal candidate will have a strong clinical background in medical-surgical, acute care, or long-term care, with experience in community-based/ambulatory care and care of the frail elderly. A Bachelor's Degree in Nursing is preferred, along with home health, hospice, palliative care, managed care, or skilled nursing facility background.

Required Qualifications:- Oregon Registered Nurse License upon hire
- 2 years Clinical medical-surgical, acute care or long-term care experience
- 1 year in community-based/ambulatory care
- 1 year in care of the frail elderly
- Bachelor's Degree in Nursing
- Home health, hospice, palliative care, managed care, or skilled nursing facility background
