About the Role:

We are seeking a skilled and compassionate Patient Support Specialist to join our team at RHA Health Services. As a key member of our interdisciplinary healthcare team, you will play a vital role in providing high-quality support to individuals with mental health or substance abuse issues in various settings.

Key Responsibilities:
  • Develop and implement personalized support plans in collaboration with our healthcare team to promote recovery, symptom reduction, and improved coping skills.
  • Establish and maintain positive relationships with clients, families, and community stakeholders to ensure seamless care coordination.
  • Provide supportive counseling and skill-building activities to enhance daily living, socialization, and behavior management skills.
  • Perform case management functions, including linking and arranging services, to ensure effective and efficient care delivery.
  • Collaborate with clinical and professional staff to maintain open communication, standardize procedures, and expedite support plan implementation.
  • Monitor service utilization to ensure it is effective, appropriate, and within established limits.
Requirements:
  • A high school diploma or equivalent is required; a GED is also acceptable.
  • A minimum of one year of documented experience working with the target population or providing similar services is preferred.
  • A valid driver's license and automotive insurance are required for positions that involve transportation.
