Key Responsibilities- Person-Centered Planning: Collaborate with the healthcare team to implement, monitor, and revise Person-Centered Plans, ensuring the highest level of functioning for our clients.
- Relationship Building: Facilitate relationships between our organization, families, local agencies, and the community, promoting a seamless support system for our clients.
- Supportive Counseling: Provide supportive counseling to minimize the negative effects of psychiatric symptoms or substance dependence, empowering clients to achieve their daily living and personal development goals.
- Skill Building: Support clients in developing essential skills, including daily and community living skills, socialization skills, adaptation skills, and behavior and anger management.
- Case Management: Perform case management functions, linking clients with necessary services and referrals, ensuring effective and efficient support.
- Communication and Collaboration: Work closely with clinical and professional staff to maintain open communication, provide feedback, and standardize procedures, expediting the implementation of Person-Centered Plans.
- Service Utilization: Monitor service utilization to ensure it is effective, appropriate, and within established limits, adhering to rules, Person-Centered Plans, and service authorizations.
- A GED or high school diploma is required.
- A minimum of one year of documented experience working with the target population or providing similar services is preferred.
- A valid driver's license and automotive insurance are required.
