  • Communication and Collaboration: Work closely with clinical and professional staff to maintain open communication, provide feedback, and standardize procedures, expediting the implementation of Person-Centered Plans.
  • Service Utilization: Monitor service utilization to ensure it is effective, appropriate, and within established limits, adhering to rules, Person-Centered Plans, and service authorizations.
Requirements
  • A GED or high school diploma is required.
  • A minimum of one year of documented experience working with the target population or providing similar services is preferred.
  • A valid driver's license and automotive insurance are required.
