Position Overview:


Delivers nursing care within the defined scope of practice to ensure that resident needs are addressed in accordance with established standards, physician directives, and regulatory guidelines.

Must hold a current license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN).

Communicate changes regarding residents to relevant departments, including room changes, leaves of absence, new admissions, and discharges.
Facilitate education for families and residents.
Identify and utilize emergency equipment as necessary.
Exhibit the ability to administer medications promptly and in accordance with facility protocols.
Exhibit the ability to perform treatments promptly and in accordance with facility protocols.

Effectively receive, transcribe, and execute physician orders, as permitted by the Nurse Practice Act. Transcribe physician orders to resident records, medication/treatment cards, and care plans as needed.

Document progress notes in a clear, factual manner that reflects the care provided and the resident's response to that care.

Adhere to established procedures for documenting and reporting incidents/accidents involving residents and staff. Recognize and report changes in resident conditions to supervisors, physicians, and family members.

Accurately identify skin changes and adhere to Solaris HealthCare skin management protocols.
Order and report diagnostic tests in a timely manner.
Conduct regular rounds to assess the effectiveness of care delivery.
Order supplies and manage charges for items ordered through Central Supply.
Follow established protocols for restraint reduction and adhere to facility policies regarding restraint use when necessary.

Ensure an adequate supply of personal protective equipment is available for staff performing procedures that involve exposure to bodily fluids.

Count narcotics at the beginning and end of each shift with the incoming and outgoing nurses.
Follow pharmacy policies and procedures for ordering and delivering medications.
Coordinate residents' rehabilitation programs with therapy services to meet rehabilitation needs.
Plan nursing interventions tailored to each resident's individual requirements and monitor their implementation.
