Position: Social Worker (SW)
Reporting To: Program Director
Role Summary:
Deliver social work and case management services to Members, collaborating with families and various stakeholders to ensure seamless care coordination.
Key Responsibilities:
- Assist with the admission process to ensure new Members are welcomed and integrated into the program effectively.
- Prepare Incident Reports in line with the guidelines set by the Managed Care Organization (MCO), state regulations, and project protocols.
- Work alongside the multidisciplinary team to formulate treatment plans tailored to the needs of identified Members.
- Conduct monthly individual sessions with Members, documenting outcomes in DAP format within 24 hours.
- Provide comprehensive case management services, including but not limited to:
- Facilitating financial eligibility and benefits coordination.
- Coordinating family engagement and support.
- Conducting utilization reviews and member care assessments with MCOs and other payers.
- Tracking member authorizations and ensuring timely input into the Electronic Health Record (EHR).
- Generating authorization reports and other necessary documentation in the EHR as required.
- Managing Level of Care adjustments and planning aftercare with Members.
- Resolving billing discrepancies promptly.
- Communicating with referral sources and external support systems (hospitals, rehabilitation centers, etc.) to facilitate Members' successful reintegration into the program.
- Assist Members with ongoing transition planning through the APPLE Plan.
- Conduct discharge follow-ups for all Members according to established processes.
- Orienting Members and families to the phone coaching process.
- Being available for coaching calls on assigned days, including emergency on-site support when necessary.
- Providing coverage for staff absences.
- Following incident reporting and documentation protocols for all on-call emergencies.
Performance Expectations:
- Engage in multidisciplinary treatment team discussions, supporting and implementing interventions as directed.
- Exhibit compassion and understanding when assisting Members, aligning with the mission and core values of Project Transition and PCS Mental Health.
- Approach Member engagement with a non-judgmental perspective, recognizing that behaviors may stem from past experiences, including trauma.
- Treat Members with dignity and respect, even in challenging situations, demonstrating supportive kindness.
- Seek guidance from Clinicians or Psychiatrists, in conjunction with the Program Director, when uncertain about how to effectively support a Member.
- Report any observations of Member behavior that may indicate a need for additional treatment or support to the Treatment Team.
- Engage with external parties and individuals professionally, embodying a positive customer service approach and representing the organization effectively.
Qualifications:
- Bachelor's degree in social work or a related field, with at least 1 year of experience in Mental Health, Behavioral Health, or Substance Use fields; or a different bachelor's degree with 2 years of relevant experience.
- A minimum of two years of experience in case management, family support, and care collaboration.
- Strong skills in assessment and documentation.
- Excellent communication, interpersonal, organizational, and time management abilities.
- Proven ability to work effectively within a multidisciplinary team.
- High-energy individual with a strong work ethic and the capacity to manage multiple tasks.
- Ability to work independently and maintain confidentiality.
