Job Summary:

This is a full-time Nurse Practitioner/APRN/Physician Assistant position offering a highly competitive salary of $112,000 - $132,000 per year. We are seeking an experienced professional with a passion for post-acute healthcare and serving the geriatric population.

About the Job:
  • As a Physiatry Specialist, you will work closely with the post-acute therapy team to optimize rehabilitative outcomes and promote patient independence.
  • You will manage conditions common in SNFs, such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries.
  • Your primary location will be in Stamford, CT, where you will provide specialized Physiatry services at a designated SNF or post-acute rehab center.
Responsibilities:
  1. Provide high-quality patient care as a rehab consultant.
  2. Represent Comprehensive Rehab Consultants through strong clinical leadership and collaboration with facility teams.
  3. Work alongside physical therapists, occupational therapists, and speech-language pathologists to improve patient function and quality of life.
  4. Foster a culture of excellence within the SNF, attending multidisciplinary meetings and bridging gaps between therapy and internal medicine.
Requirements:
  • Nurse Practitioner/APRN/Physician Assistant licensure in Connecticut.
  • Ability to work autonomously and navigate complex healthcare environments.
  • Strong interpersonal skills, empathy, and a genuine interest in delivering exceptional patient care.
