About the Role:

We are seeking an experienced Speech Language Pathologist to join our team as a Neuro Rehabilitation Specialist. As a key member of our Rehab Without Walls program, you will provide functional rehabilitation services to patients in their natural home and community environment.

Responsibilities:

  • Perform evaluations and develop treatment plans for patients with speech, language, cognitive, and swallowing disorders.
  • Regularly reassess treatment plans, attend staffing meetings, and family conferences.
  • Perform discharge evaluations, make recommendations for home and community safety, and provide referral and education resources.
  • Communicate patient needs and progress to the treatment team, physician, and family members.
  • Collaborate with other disciplines to ensure coordination of care and enhance patient outcomes.

Requirements:

  • Master's degree from an accredited Speech Language Pathology program.
  • Valid Speech Language Pathology license in the state(s) of practice.
  • Current CPR certification in accordance with state regulations.
  • Minimum of one year's work experience as a Speech Language Pathologist.
  • Demonstrated knowledge of rehabilitation techniques related to complex neurological injury.
