Job Overview

Evergreen Life Services is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team. As an LPN, you will play a vital role in providing high-quality healthcare services to individuals with intellectual and developmental disabilities (IDD).

Responsibilities
  • Contribute to the planning and implementation of daily healthcare projects
  • Update medical staff on health-related issues and discuss treatment plans
  • Assist in educating staff on individual health needs and conditions
  • Participate in individual support plan (ISP) development and implementation
  • Monitor and report on individual health status, including vital signs and medication administration
  • Collaborate with medical staff to ensure accurate and timely documentation
  • Provide transportation for individuals as needed
Requirements
  • Graduate of an approved LPN program
  • Minimum one year of clinically relevant experience, preferably in IDD or geriatric care
  • Knowledge of medical nursing procedures, first aid, and basic principles of nursing
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
Working Environment

As an LPN at Evergreen Life Services, you will work in a variety of settings, including group homes and adult day care centers. You will be required to travel frequently and work with individuals with IDD, their families, and other healthcare professionals.
