Job Summary

We are seeking a highly skilled Care Coordinator to join our team at Haven Behavioral Healthcare. This is a part-time position, working every other Saturday day shift.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. They will be responsible for receiving and responding to call inquiries, obtaining necessary information for admissions, making referrals as needed, and completing pre-authorizations of services.

This role offers a unique opportunity to make a meaningful difference in the lives of individuals struggling with mental health and substance abuse issues. As a Care Coordinator, you will play a critical role in ensuring that patients receive the highest level of care and support.

About the Role

This position is based in our new 72-bed facility in West Chester, PA, providing high-quality behavioral health services to individuals and families in need. Our team is passionate about delivering compassionate, quality care, and we are committed to fostering a culture of service and excellence.

You will have the opportunity to work closely with multidisciplinary teams, including medical professionals, therapists, and support staff, to provide comprehensive care and support to patients.

Key Responsibilities
  • Receive and respond to call inquiries for potential admissions and general questions
  • Obtain necessary information to facilitate admissions
  • Makes referrals for individuals who are not suitable for admission
  • Complete pre-authorizations of services
Requirements

To be successful in this role, you will require a High School Diploma or equivalent, with a Bachelor's Degree preferred. Experience in crisis management or intake is also desirable.

Certifications and licenses such as CPI/CPR per policy are required. Current licensure or certification by discipline is preferred.

We offer a competitive salary of $45,000 - $55,000 per annum, depending on experience, as well as a range of benefits and opportunities for professional growth and development.
