We are seeking an experienced Hospice & Palliative Care Nurse Practitioner to join our team.
- Implement palliative care program requirements and complete medical visits with patients according to guidelines.
- Coordinate care with the Palliative Care team as clinically indicated and provide re-certification information to the Medical Director/Associate Medical Director.
- Communicate with patients, families, and agency personnel at all levels.
Requirements:
- Master's degree in Nursing with current licensure in the state of practice.
- Certification as an Adult or Family Nurse Practitioner.
- Experience in Hospice or Palliative Care nursing preferred.
- Knowledge of the framework, function, and organization of a hospice care program.
