Job Title: Hospice Registered Nurse

Job Summary:

Liberty Cares is seeking an experienced Hospice Registered Nurse to join our team. As a Hospice Registered Nurse, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to deliver compassionate care to our patients and their families.

Responsibilities:

  • Apply nursing theories and concepts to provide patient-centered care.
  • Coordinate physical care of patients with family, patient, physician, and Hospice team.
  • Communicate effectively with patients, families, and healthcare team members.
  • Participate in IDT meetings and quality reviews as scheduled by the agency.

Requirements:

  • Current RN licensure in the state of practice.
  • Successful completion of a nursing education from an approved school of nursing.
  • Two years of clinical nursing experience and at least one year of hospice nursing experience preferred.
  • Knowledge of the framework, function, and organization of a home health and hospice care program.
  • Working knowledge of nursing principles and practice according to nursing practice act, agency policies, and procedures.
  • 12 hours of continuing education annually.
  • CPR certification.
  • Ability to accept travel assignments and weekend/evening and holiday call assignments.
