We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Community Options, Inc. in Mercer County, NJ.

Job Summary:

The LPN will provide high-quality nursing care to individuals with disabilities in a community setting, utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation.

Responsibilities:
  • Provide treatment and healthcare procedures ordered by a physician and required by the standards of professional practice or law.
  • Ensure individuals attend medical appointments per their Individual Support Plan (ISP).
  • Advise supervisor of all concerns and issues related to the provision of nursing services.
  • Create and distribute medical/health reports detailing general health, health care goals, and physician recommendations.
  • Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team to include any concerns, observations, and behavioral developments.
  • Train employees on the medical needs of assigned individuals.
  • Monitor health and medical conditions of individuals and refer for medical services as needed.
  • Monitor the use of medications through medication administration logs.
  • Ensure health service supplies are available and replenished as needed.
  • Ensure program documentation and billable records are completed accurately and timely.
  • Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families.
  • Participate with individuals in community, social, and recreational activities.
  • Participate in circles of support for personal futures planning.
  • Follow strict OSHA procedures regarding blood borne pathogens.
  • Employees must cooperate with the licensee and department staff in any inspection, inquiry or investigation.
Requirements:
  • Bachelor's Degree or equivalent experience in nursing field.
  • Minimum of one year of experience providing nursing services to individuals with disabilities.
  • Valid nursing license.
  • Valid driver's license with a satisfactory driving record.
  • Complete all state and agency required training per state guidelines.
  • Ability to calculate figures related to medication dosages.
  • Understanding and commitment to community-based support for persons with disabilities.
  • Ability to exercise good judgment and remain calm in crisis situations.
  • Ability to establish a comfortable and supportive relationship with an individual receiving supports.
  • Proficient with Microsoft Office.
  • Excellent verbal and written communication skills.
  • Ability to effectively present facts and recommendations.
  • Ability to effectively communicate with program participants, third-party agencies, staff and members of the community.
  • Excellent time management skills.
Working Conditions:
  • Work in office, residential programs, day programs, and community environments.
  • May be exposed to changing weather conditions when working outside.
  • Hours and weekly schedules may change due to business needs and may include evening and weekend hours.
  • May be required to be on-call in cooperation with other management staff.
  • Occasional overtime may be required.
  • Frequent lifting, stretching, and other physical exertion may be required.
  • May be required to lift or move 50+ pounds.
  • May occasionally assist with wheelchair transfer of non-ambulatory individuals.
  • May be required to transport individuals utilizing your own vehicle or company provided vehicles.
  • May be exposed to various medical conditions, communicable diseases, blood, body fluids, or tissues.
