Home Care Registered Nurse Full Time Opportunity

Amedisys, a leading provider of home health, hospice, and high-acuity care, is seeking a compassionate and skilled Registered Nurse (RN) to join our team as a Case Manager in home care. As a RN Case Manager, you will have the opportunity to make a meaningful impact on the lives of patients and their families by providing much-needed care in the comfort of their own homes.
Responsibilities:- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Makes referrals to other disciplines as indicated by the patient's identified needs or documents rationale for not doing so.
- Promotes patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques.
- Regularly communicate patient progress to the clinical manager and care team.
- Plans and provides staff education.
