We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Case Manager in Homecare. As a key member of our care team, you will have the opportunity to make a meaningful impact on the lives of patients and their families by providing much-needed care in the comfort of their own homes.
Responsibilities- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Makes referrals to other disciplines as indicated by the patient's identified needs or documents rationale for not doing so.
- Promotes patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques.
- Regularly communicate patient progress to the clinical manager and care team.
- Plans and provides staff education.
- A full benefits package with choice of affordable PPO or HSA medical plans.
- Up to $1,300 in free healthcare services paid by Amedisys yearly, when enrolled in an Amedisys HSA medical plan.
