Job Opportunity

Pinnacle Home Care Inc. is seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join our team of skilled home health care providers. As a PTA, you will play a vital role in providing quality care to our patients in their homes.

Key Responsibilities:

  • Assist in the evaluation and development of a rehabilitative plan of care and periodic re-evaluation as required.
  • Participate in the preparation of clinical and daily progress notes, all notes are reviewed by the Physical Therapist and submitted according to company policy.
  • Perform routine strengthening and therapeutic exercises and assist patients with the use of self-help devices.
  • Instruct patients and family members on exercise programs and provide guidance on the use of assistive devices.
  • Identify potential medication issues and report them to the Physical Therapist.
  • Record activities and findings in the clinical record and report to the Physical Therapist and physician any adverse reactions to treatment.
  • Consult with the Physical Therapist regarding the outcome of home visits and report physical findings.
  • Collaborate with other healthcare team members, including Home Health Aides, to ensure comprehensive care.
  • Prepare daily progress notes and submit them according to company policy.
  • Participate in discharge activities and assist the Physical Therapist in completing the Physical Therapy Discharge Summary.
  • Confirm visit scheduling with the Supervisor/Director of Nursing to coordinate Agency visits.

Requirements:

  • Graduate of an accredited Physical Therapy Assistant program or equivalent education.
  • Licensed to practice as a PTA in the State of Florida.
  • Current BLS Certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • Minimum of one year's experience in home healthcare preferred.
  • Electronic medical record experience is preferred but not essential.
  • Meets all qualifications as stated within the state practice act. Must possess and maintain current certification in cardiopulmonary resuscitation.
  • Must display good emotional health and be able to physically tolerate much standing, stooping, bending, and heavy lifting; must meet all agency regulations.
  • Must be self-directed and able to work with little supervision and have good observational, judgment, and communication skills.
