Job Title: Case Manager for Mental Health

Sea Mar Community Health Centers is seeking a dedicated and compassionate Case Manager for Mental Health to join our team in Seattle, South Park. As a Case Manager, you will provide primary community support and case management to mentally ill, homeless, and substance abuse clients, working closely with our Behavioral Health Program.

Key Responsibilities:
  • Conduct intake screenings to determine client needs and develop individualized service plans.
  • Provide advocacy and support to clients, including home visits and referrals to community resources.
  • Maintain accurate records and participate in regular interdisciplinary team case reviews.
  • Collaborate with other staff members to provide comprehensive services and promote a positive team approach.
Requirements:
  • Associate's degree in Behavioral Health, Health Sciences, or related field, and two years of closely related work experience.
  • Eligibility for Agency Affiliated Counselor Registration from the State Department of Health.
  • Ability to speak effectively before groups and write routine reports and correspondence.
  • Bilingual, fluent in Spanish and English, proficient in reading and writing in both languages.
