Job Summary

We're seeking a skilled Patient Advocate and Behavioral Health Specialist to join our team. This individual will work closely with patients and families to enhance their psychosocial functioning, providing case management and therapy services to promote overall well-being.

Responsibilities
  • Provide case management and therapy services to patients and families, focusing on enhancing their psychosocial functioning.
  • Work collaboratively with interdisciplinary teams to ensure comprehensive care.
  • Develop and implement personalized treatment plans tailored to each patient's needs.
  • Maintain accurate and timely documentation of patient progress and outcomes.
Qualifications

To be successful in this role, you'll need:

  • A Master's degree in Social Work, Counseling, or a related field.
  • Licensure as a Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), or Licensed Professional Counselor (LPC).
  • A minimum of 2-3 years of experience in behavioral health, preferably in a hospital setting.
