About the Role
A Behavioral Health Care Specialist at Affinia Healthcare is a vital member of our team, providing direct behavioral health care in collaboration with medical providers to integrate physical and behavioral healthcare needs of patients.
Key Responsibilities
- Meet daily with the medical team for huddles to discuss patient care plans.
- Engage with patients in exam rooms to provide behavioral interventions, assessments, and support.
- Develop and implement health self-management goals with patients, addressing social and emotional factors related to their medical conditions.
- Collaborate with primary care providers and psychiatrists as needed to coordinate treatment.
- Provide crisis intervention, risk assessment, and follow-up planning for patients referred by their PCP.
- Screen patients for appropriate psychiatry referral and offer consultation services to other Affinia Healthcare programs.
Requirements and Qualifications
- Masters degree in Social Work required.
- Licensed Clinical Social Worker (LCSW) preferred, but LMSW and LPC candidates will be considered.
- Three years of experience in a health/mental health setting as a LCSW, including two years in an under-served community.
- Highly skilled in problem-solving, familiar with DSM V diagnostic criteria, and proficient in basic counseling skills.
- Strong communication and computer skills, including electronic medical record familiarity.
