Job Title: Licensed Practical Nurse LPN

Job Summary:

The Licensed Practical Nurse LPN will be responsible for providing high-quality patient care in a fast-paced healthcare environment. This role will involve working closely with the healthcare team to develop and implement individualized care plans, administer medications, and provide emotional support to patients and their families.

Key Responsibilities:

  • Assessment and Care Planning: Conduct thorough assessments of patients to identify their physical, emotional, and social needs. Develop and implement individualized care plans that address these needs and promote optimal patient outcomes.
  • Medication Administration: Administer medications as prescribed by physicians and in accordance with established policies and procedures.
  • Wound Care and Management: Provide wound care and management services to patients, including dressing changes, wound cleaning, and debridement.
  • Emotional Support: Provide emotional support and comfort to patients and their families, including counseling and education on disease management and self-care.
  • Communication: Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination and patient satisfaction.
  • Documentation: Maintain accurate and timely documentation of patient care, including progress notes, medication administration records, and other relevant information.
  • Team Collaboration: Work collaboratively with the healthcare team to ensure that patient care needs are met and that the care plan is implemented effectively.
  • Quality Improvement: Participate in quality improvement initiatives to identify areas for improvement and implement changes that promote high-quality patient care.

Requirements:

  • Education: Graduate of an accredited Licensed Practical Nurse LPN program.
  • Experience: Minimum of 1 year of experience as a Licensed Practical Nurse LPN in a healthcare setting.
  • Skills: Strong assessment and communication skills, ability to work effectively in a fast-paced environment, and proficiency in electronic medical records.
  • Certifications: Current LPN licensure in the state of employment.

The Licensed Practical Nurse LPN will work in a fast-paced healthcare environment, providing patient care in a variety of settings, including medical-surgical units, intensive care units, and rehabilitation units. The ideal candidate will be able to work effectively in a team environment, communicate effectively with patients and families, and provide high-quality patient care in a timely and efficient manner.
