Job Summary

We are seeking a highly skilled and compassionate Social Services Assistant to join our team at PACS Inc. As a key member of our social services department, you will play a vital role in ensuring the well-being and quality of life for our residents.

Key Responsibilities
  • Assist the Social Service Director in planning, developing, and implementing social service programs.
  • Meet with administration, medical, and nursing staff to plan social services and develop department policies and procedures.
  • Review and update department policies and procedures annually and participate in making recommended changes.
  • Assist in developing and implementing policies and procedures for identifying medically related social and emotional needs of residents.
  • Participate in community planning related to the interests of the facility and the services and needs of residents and families.
  • Assist residents in achieving the highest practicable level of self-care, independence, and well-being.
  • Provide medically related social services to ensure the highest practicable physical, mental, and psychosocial well-being of each resident.
  • Perform administrative requirements, such as completing necessary forms and reports, and submitting them to the Director.
  • Provide information to residents and families about Medicare, Medicaid, and other financial assistance programs available to residents.
  • Record and maintain regular social service progress notes indicating response to the treatment plan and/or adjustment to institutional life.
  • Coordinate social service activities with other departments as necessary.
Requirements
  • Must possess a high school diploma or its equivalent; Associate or Bachelor's Degree preferred but not required.
  • Prior healthcare experience preferred.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
