Aftercare Clinician

Elmcrest Children's Center is seeking a skilled Aftercare Clinician to provide clinical services to youth and families transitioning from our residential program to the community. As a key member of our team, you will be responsible for delivering individual, group, and family therapy, as well as communicating with various stakeholders to ensure seamless support.
Key Responsibilities:- Provide clinical services to youth and families in their home and community settings.
- Collaborate with families, county representatives, and other service providers to ensure comprehensive support.
- Develop and implement treatment plans, lethality and risk assessments, and other clinical paperwork.
- Meet minimum required billables per week.
- Master's Degree or Licensed Professional in mental health field (LMSW, LMFT, LMHC, etc.)
- Experience working in mental health environment
- Knowledge of community resources and support systems
- Ability to work independently and collaboratively with a multidisciplinary team
- Valid Driver's License, proof of automobile insurance
