Aftercare Clinician

Elmcrest Children's Center is seeking a skilled Aftercare Clinician to provide clinical services to youth and families transitioning from our residential program. As a key member of our team, you will play a vital role in ensuring a smooth transition for our youth and their families.
Job Summary:The Aftercare Clinician will provide individual, group, and family therapy within the community, maintaining consistent contact with family members, other service providers, and county representatives. This role requires a strong understanding of community resources and support systems, as well as the ability to work independently and collaboratively with a multidisciplinary team.
Key Responsibilities:- Provide clinical services to youth and families, including individual, group, and family therapy
- Maintain consistent contact with family members, other service providers, and county representatives
- Complete clinical paperwork and assessments, such as treatment plans and lethality and risk assessments
- Meet minimum required billables per week
- Collaborate with the Aftercare Coordinator to ensure seamless transition of services
- Master's Degree or Licensed Professional in mental health field (LMSW, LMFT, LMHC, etc.)
- Experience and familiarity working with/in mental health environment
- Knowledge of community resources and support systems
- Ability to work independently and to collaborate effectively with a multidisciplinary team
- Valid Driver's License, proof of automobile insurance
