Responsibilities

  • Provide high-quality, patient-centered care to children with complex medical needs in the comfort of their own homes.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Conduct thorough assessments, administer medications, and perform various medical procedures as needed.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments.

Requirements

  • Current RN or LPN licensure in the state of Minnesota.
  • Experience working with pediatric patients, preferably in a home care setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a focus on delivering exceptional patient care.
  • Flexibility to work a variety of shifts, including days, evenings, and overnights.
