Key Responsibilities:
- Conduct comprehensive medical assessments for patients entering the Skilled Nursing Facility.
- Assess and document the medical necessity for various levels of care, facilitating patient transfers when required.
- Provide guidance and support to nursing staff and referring physicians regarding medical interventions aligned with the care plan.
- Collaborate with attending physicians and referral sources to ensure optimal medical care for all patients.
- Engage with patients and their families to discuss medical conditions and care strategies.
- Contribute to the development and review of clinical protocols within the facility.
- Communicate outcomes of consultations and evaluations to primary care physicians and referral sources.
- Maintain thorough documentation of interactions with nursing staff, referral sources, patients, and families.
- Establish and maintain regular communication with assigned referral sources.
- Complete and submit accurate clinical notes regarding patient conditions and care plans.
- Utilize electronic health records to ensure high-quality care, accuracy, and effective communication.
- Provide educational resources and information to the care team and medical community as necessary.
- Assist in resolving clinical or ethical dilemmas and interpersonal conflicts.
- Participate in research initiatives or special projects as assigned.
- Be part of the on-call rotation with other team members (additional compensation provided).
- Perform additional duties as needed.
Qualifications:
- Experience:
- At least two (2) years of professional experience in the healthcare sector.
- Experience in a skilled nursing environment is preferred.
- Experience in acute care settings is advantageous.
- Education and Training:
- Current Nurse Practitioner or Physician Assistant licensure is mandatory.
- Licensure and Certification:
- Must hold and maintain a valid CPR Certification.
- Current licensure as a Nurse Practitioner or Physician Assistant in Massachusetts is required.
- Possession of a valid driver's license.
- Additional Requirements:
- Exhibit excellent verbal and written communication skills, along with strong organizational abilities.
- Demonstrate keen observation and problem-solving capabilities.
- Experience working within an Interdisciplinary Team is preferred.
Working Environment:
- Potential exposure to infections and contagious diseases.
- Direct contact with patients in various circumstances.
- Subject to unpredictable situations and emergency scenarios.
- Travel may be required.
- OSHA exposure category: Category I (tasks involve exposure to blood, body fluids, tissues).
