Job Title: Breast Nurse Navigator

Join Radiology Imaging Associates as a Breast Nurse Navigator and play a vital role in providing comprehensive care to patients diagnosed with breast cancer and/or high-risk breast conditions.

Job Summary:

The Breast Nurse Navigator will work collaboratively with the breast health team to provide seamless care to patients, ensuring optimal outcomes. This role requires strong communication and interpersonal skills, as well as the ability to navigate patients through various multidisciplinary breast care programs.

Key Responsibilities:
  • Coordinate care for newly diagnosed patients, including those with breast cancer, high-risk, and benign findings.
  • Collaborate with breast radiologists to inform referring physicians about diagnoses and confirm orders for additional imaging/procedures and referrals to specialists.
  • Call patients with breast cancer or high-risk lesions to inform them of their biopsy results and provide education about the immediate diagnosis and plan for subsequent diagnostic evaluation.
  • Follow up with newly diagnosed patients to discuss the nurse navigation role, confirm/schedule additional appointments, and address concerns.
  • Provide continual support to newly diagnosed patients through subsequent diagnostic evaluations and recommendations for further biopsies.
  • Assist with communication among the multi-disciplinary team during this process.
  • Call patients with benign biopsy results.
  • Assist with nursing duties as needed, including checking and monitoring vital signs, blood draws, wound care, and other responsibilities.
  • Respond to emergency care as needed within the radiology department.
  • Closely collaborate with the Sarah Cannon nurse navigator team and provide a smooth transition for patients.
  • Perform follow-up calls to patients post-biopsy with positive and benign results.
  • Assist patients and their families in overcoming barriers by providing information and referrals to internal and external resources.
  • Build longitudinal relationships with referring physicians and document their preferences related to orders for additional imaging/procedures and referrals to specialists.
  • Educate referring physician offices about HealthOne-accredited multi-disciplinary programs.
  • Document all patient contacts, treatment providers, appointment dates, surgical procedures, and treatment updates using the mammography reporting system.
  • Attend relevant case conferences and administrative meetings on a regular basis.
  • Actively participate in community outreach programs and activities.
  • Work collaboratively with the team to develop, evaluate, and improve relevant processes.
Requirements:
  • Advanced knowledge of breast cancer screening and diagnostic tools.
  • Proven success in a protocol/pathway-driven environment.
  • Able to navigate patients through various multidisciplinary breast care programs.
  • Able to assess, plan, intervene, and evaluate the physical, emotional, and educational needs of patients.
  • Strong written/verbal communication and interpersonal skills to effectively interact with patients, co-workers, referring physician groups, and radiologists.
  • Excellent problem-solving and organizational skills.
  • Able to work efficiently and effectively both independently and as a member of a team.
  • Strong computer and systems skills.
  • Able to manage multiple duties simultaneously in a fast-paced environment.
Education Requirements:
  • Graduate of an accredited school of Nursing; valid registration as a Registered Nurse in the State of Colorado.
  • CBPN-IC NCBC, AAMSN, and ACLS certifications preferred.

Compensation for this role is between $75,000.00 and $102,200.00 per year based on experience. Radiology Imaging Associates offers a comprehensive benefits package, including medical, dental, vision, term life insurance, AD&D, EAP, Short Term Disability, Long Term Disability, PTO, and holiday pay for eligible employees. Part-time employees may have access to some of these benefits, which may be on a pro-rated basis. Full-time employees will become eligible for benefits on the first of the month following 30 days of employment.
