Job Summary

The Breast Nurse Navigator is a key member of the Radiology Imaging Associates team, responsible for providing comprehensive care to patients diagnosed with breast cancer and/or high-risk breast conditions. This role requires strong communication and interpersonal skills, as well as the ability to navigate patients through various multidisciplinary breast care programs.

Key Responsibilities
  • Coordinates care for newly diagnosed patients, including those with breast cancer, high-risk, and benign findings.
  • Collaborates with breast radiologists to inform referring physicians about diagnoses and confirm orders for additional imaging/procedures and referrals to specialists.
  • Provides education about immediate diagnoses and plans for subsequent diagnostic evaluations, which may include additional imaging, surgical/medical oncology consultations, and genetic counseling.
  • Follows up with newly diagnosed patients to discuss the nurse navigation role, confirm/schedule additional appointments, and act as a resource to answer questions and address concerns.
  • Assists with communication among the multi-disciplinary team during the diagnostic process.
  • Responds to emergency care as needed within the radiology department.
  • Closely collaborates with the Sarah Cannon nurse navigator team and provides a smooth transition for patients.
  • Performs follow-up calls to patients post-biopsy with positive and benign results.
  • Assists patients and their families in overcoming barriers by providing information and referrals to internal and external resources.
  • Builds longitudinal relationships with referring physicians and documents their preferences related to orders for additional imaging/procedures and referrals to specialists.
  • Educates referring physician offices about HealthOne-accredited multi-disciplinary programs.
  • Documents all patient contacts, treatment providers, appointment dates, surgical procedures, and treatment updates using the Mammography Reporting System.
  • Attends relevant case conferences and administrative meetings on a regular basis.
  • Actively participates in community outreach programs and activities.
  • Works collaboratively with the team to develop, evaluate, and improve relevant processes.
Requirements
  • Graduate of an accredited school of nursing with valid registration as a Registered Nurse in the State of Colorado.
  • CBPN-IC NCBC, AAMSN, and ACLS certifications preferred.
  • Advanced knowledge of breast cancer screening and diagnostic tools.
  • Proven success in a protocol/pathway-driven environment.
  • Able to navigate patients through various multidisciplinary breast care programs.
  • Able to assess, plan, intervene, and evaluate the physical, emotional, and educational needs of patients.
  • Strong written/verbal communication and interpersonal skills to effectively interact with patients, co-workers, referring physician groups, and radiologists.
  • Excellent problem-solving and organizational skills.
  • Able to work efficiently and effectively both independently and as a member of a team.
  • Strong computer and systems skills.
  • Able to manage multiple duties simultaneously in a fast-paced environment.
