Primary Purpose
We are seeking a certified Nurse Practitioner to join our Advantmed provider network. As a risk adjustment specialist, you will conduct in-home wellness assessments for Medicare members and other at-risk populations. Your primary objective will be to assess the overall health and well-being of members to ensure accurate and comprehensive risk adjustment coding, leading to greater value-based care.
NP Responsibilities
As a certified Nurse Practitioner, you will be responsible for:
- Performing annual in-home wellness visits and risk adjustment health assessments on populations with chronic conditions.
- Delivering patient health education on topics such as pain management, medication, and daily living activities.
- Building relationships of trust with members through exceptional communication and empathy.
- Assisting in closing quality care gaps, such as screenings and labs.
- Contributing to the enhancement of care delivery through collaboration and innovation.
Requirements
To be considered for this role, you must have:
- A valid unencumbered NP license for the state where you will be working.
- Previous in-home risk assessment experience is preferred.
- Previous 1099 (PRN) experience is preferred but not required.
- Access to reliable transportation to travel to members' homes within a designated area.
- Strong ability to work within our EMR system.
- Ability to work independently.
