Clinical Case Manager

This position is designated for Licensed Registered Nurses. Under general supervision, this role involves mentoring new or less experienced case managers, aiding their professional development and ensuring their effectiveness in case management. The role requires a comprehensive understanding of relevant principles and practices and entails performing additional clinical tasks typical of an RN, including the assessment of critical need or high-risk patients to ensure appropriate care is prescribed and delivered. Participation in case management programs necessitating clinical evaluations is also a key responsibility.
Key Responsibilities:
- Provide guidance to caregivers, deliver patient education and counseling, and facilitate referrals for additional services.
- May engage in care and case management or interventions aimed at promoting health, preventing illness, and overcoming barriers to healthcare access.
- Gather detailed information regarding a client's circumstances and functioning through interviews and other pertinent sources to identify specific needs.
- Formulate a comprehensive case management strategy addressing identified needs.
- Monitor and assess treatment plans and patient progress.
- Encourage interdisciplinary collaboration.
- Evaluate staff performance.
- Develop and execute actionable, time-sensitive case management plans by establishing and documenting clear objectives, goals, and actions tailored to meet the client's health and human service requirements identified during the assessment phase.
- Organize, secure, integrate, modify, and document the resources necessary to achieve the goals outlined in the case management plan.
- Collect information and documentation from all relevant sources regarding the activities and/or services of the case management plan to evaluate its effectiveness in achieving desired outcomes and objectives.
- Adjust or revise the case management plan as necessary.
- Measure and analyze interventions to assess the outcomes of case management involvement, including clinical, financial, variance, quality of life, and client satisfaction metrics.
- Perform additional duties as required to effectively fulfill the responsibilities of the position.
Job Requirements
Required Education: Bachelor's Degree in Nursing.
- Minimum of 12 months of professional nursing experience.
Essential Skills:
- In-depth knowledge of federal and state regulations, particularly concerning patient confidentiality.
- Understanding of case management fundamentals and the technical/procedural aspects involved.
- Familiarity with medical terminology.
- Ability to resolve moderate to complex issues through sound judgment, critical thinking, and problem-solving skills.
- Strong organizational skills to prioritize, manage, and complete projects or documentation efficiently.
- Capability to maintain accurate and detailed records.
- Ability to manage stressful situations and adapt to varying circumstances.
- Proficiency in setting priorities and navigating obstacles while remaining focused on objectives.
- Skill in building and sustaining professional relationships with clients, colleagues, and other organizations while adhering to ethical standards.
- Self-motivated with a drive to achieve results, adaptable to necessary changes, and responsive to constructive feedback.
- Ability to analyze information and evaluate results to determine the appropriate course of action while adhering to established standards.
- Capacity to develop and maintain collaborative working relationships.
Certifications:
- State Registered Nurse Certification.
Physical Requirements:
- Physical: Ability to sit for extended periods. Effective communication skills for speaking and listening.
