Key Responsibilities- Case Management: Develop and implement comprehensive case management plans to address the unique needs of patients, ensuring timely and effective care coordination.
- Clinical Leadership: Mentor and guide new or lower-level case managers, providing guidance and support to ensure their success in the role.
- Collaboration and Communication: Foster strong relationships with healthcare providers, patients, and families to ensure seamless care transitions and optimal patient outcomes.
- Quality Improvement: Analyze data and evaluate treatment plans to identify areas for improvement, implementing changes to enhance patient care and outcomes.
- Staff Development: Provide training and education to staff members on case management best practices, ensuring a high level of competence and confidence in their roles.
- Education: Bachelor's Degree in Nursing, with a minimum of 12 months of professional nursing experience.
- Certifications: State of Oklahoma Registered Nurse Certification.
- Skills: Strong knowledge of case management fundamentals, medical terminology, and federal and state laws related to patient confidentiality. Excellent communication, leadership, and problem-solving skills.
