We are seeking a highly skilled and detail-oriented Medication Reconciliation Technician to join our team at Auburn Community Hospital. As a Medication Reconciliation Technician, you will play a critical role in ensuring the accuracy and safety of patient medication lists.
Key Responsibilities- Collect and enter patient medication histories into the hospital's information system.
- Communicate with healthcare professionals to maintain adequate workflow and identify patient-specific needs.
- Accurately enter home medication lists into the hospital's information system for utilization by providers.
- Direct unresolved issues to pharmacists and assist with medication management tasks.
- Perform other duties as assigned to support the pharmacy team.
- High school graduate with certified pharmacy technician license or one year of pharmacy experience.
- Ability to interpret and perform mathematical computations.
- Must obtain and maintain certified pharmacy technician certificate from a nationally recognized program within one year from date of hire.
- Must obtain and maintain a current NYS Pharmacy Technician License within one year from date of hire.
This is a part-time position with a 30-hour workweek. The expected schedule includes 10-hour shifts and weekends as needed. The work location is in-person at Auburn Community Hospital.
