Position Overview: AuthoraCare Collective is in search of a dedicated Full-time Speech Therapist to join our Integrated Health Services team. This role involves providing skilled home-based care across various service areas.

Key Responsibilities:

  • Travel to patient residences within the designated service areas to deliver therapy services.
  • Implement speech and language therapy in alignment with the patient's care plan as prescribed by their primary care provider.
  • Collaborate with the interdisciplinary team to manage the speech therapy Plan of Treatment for a caseload of Integrated Health patients.
  • Conduct assessments to identify patient needs, set therapy objectives, and determine appropriate interventions.
  • Work closely with physicians to create comprehensive treatment plans utilizing diverse methods, including therapeutic exercises and hands-on techniques.
  • Foster and maintain a therapeutic relationship with patients, offering community support and educational resources.
  • Document therapy goals and outcomes effectively, including evaluations and treatments for hospice patients when necessary.

Qualifications:

  • Master's or doctoral degree in speech-language pathology.
  • A minimum of one year of experience in speech therapy, preferably in home health or facility settings.
  • Current and unrestricted Speech Therapy Licensure.
