We are seeking skilled Licensed Vocational Nurses (LVNs) to join our team at Continuity Care Home Nurses, a reputable home health agency providing private duty nursing services in Southern California.
Job Summary:
- Provide high-quality patient care to individuals with complex medical needs, including those with brain injuries, paralysis, and ventilator dependence.
- Collaborate with registered nurses to administer medications and develop patient care plans.
- Conduct thorough assessments, educate patients and families on medical treatments, and maintain accurate documentation.
Responsibilities:
- Deliver basic patient care and treatment, including wound care, vital sign monitoring, and medication administration.
- Work collaboratively with the interdisciplinary team to ensure seamless patient care.
- Participate in ongoing education and training to stay current with industry developments and best practices.
Requirements:
- One year of clinical experience, preferably in a facility or rehabilitation setting.
- Current LVN licensure and CPR certification.
- Valid driver's license, auto insurance, and a clean driving record.
- Physical and TB test, as well as two work references and valid I9 documentation.
- Compassionate and caring demeanor, with excellent communication and interpersonal skills.
