Job Title: Case Manager

Alleghany Highlands Community Services is seeking a skilled Case Manager to provide clinical, case management, and consultation services to individuals with primary mental illness who are currently incarcerated.

Responsibilities:
  • Develop and maintain working relationships with individuals served, jail staff, drug court staff, and other community agencies and resources to ensure quality services.
  • Provide services to mentally ill and co-occurring individuals in incarcerated settings.
  • Aid in transitioning consumers who are released from local jails to arrange for services with AHCS, if services are desired by the consumer and/or required to maintain the consumer's stability in the community.
  • Establish and maintain a community referral network and advocate for services on behalf of individuals and their families.
Requirements:
  • Bachelor's Degree in a Human Services field.
  • Meet Virginia requirements for a Qualified Mental Health Professional or Qualified Mental Health Professional Trainee.
  • Valid Driver's License.
