Job Summary

As a Nurse Care Coordinator at Williamson Health, you will play a vital role in coordinating and implementing comprehensive nursing care for patients across various units. This position involves collaborating with healthcare teams, managing patient assessments, and ensuring adherence to medical standards while providing compassionate care.

We offer comprehensive in-patient and out-patient services, with more than 825 providers representing over 70 medical specialties and sub-specialties.

Responsibilities

  1. Performs a comprehensive assessment on all patients within the appropriate time of admission utilizing skills of observation, communication, and examination; including data obtained from patient, family, and other members of the healthcare team.
  2. Plans the patient's care on an individual basis, identifying problems/needs, nursing diagnosis, standards of care/practice, interventions consistent with identified problems, and attainable outcomes.
  3. Plans for patient teaching, discharge, and post-discharge needs as appropriate for the patient population.
  4. Functions independently with treatments, procedures, and equipment appropriate to areas, evidenced by adherence to policy, procedure, and competency-based assessment and no pattern or trend of problem.
  5. Handles/transports/stores/prepares/administers medications/solutions safely and in accordance with hospital and departmental policies and procedures.
  6. Documents accurately and timely.
  7. Reassesses patient's condition, evaluates patient response, and modifies plan of care accordingly and in a timely manner.
  8. Accepts charge responsibilities and assigns staff with consideration of patient condition and care requirement, acuity, and complexity of patient's condition. Facilitates staff development.
  9. Maintains a healing and therapeutic environment for the patient and family.
  10. Maintains position-specific qualifications.
  11. Demonstrates responsibility for assigned communications and special assignments.
  12. Maintains their work areas and personal appearance in a way that reflects our professionalism and demonstrates to our patients, visitors, and coworkers the importance we place on cleanliness, orderliness, and safety.

Requirements

  • Earned a nursing diploma or degree from a college or university.
  • Licensed in the State of Tennessee.
  • BLS within 90 days of hire.
  • For 3rd-floor telemetry unit: Basic arrhythmia within 6 months of hire and ACLS within 6 months of hire.
  • For float pool and float pool extended: Basic arrhythmia within 6 months of hire, ACLS within 6 months of hire, and Non-Violent Crisis Intervention (NVCI) certification within 6 months of hire.
  • For critical care: Basic arrhythmia within 6 months of hire, ACLS within 6 months of hire, Non-Violent Crisis Intervention (NVCI) certification within 6 months of hire, and AACN, CCRN, or CCP certification within two years of hire.
  • For Oncology: ONS chemo/immunotherapy certification within one year of hire.
  • For Adult Emergency Room: At least one year of emergency experience preferred, TNCC Preferred, ACLS within 6 months of hire, PALS within 6 months of hire, and Non-Violent Crisis Intervention (NVCI) certification within 6 months of hire.
  • For Pediatric Medical Surgical: At least one year of pediatric in-hospital experience preferred, PALS within 6 months of hire, and Non-Violent Crisis Intervention (NVCI) certification (8-hour, non-physical training) within 6 months of hire.
  • For Pediatric Emergency Room: At least one year of pediatric emergency experience preferred, PALS within 6 months of hire, ACLS within 6 months of hire, Non-Violent Crisis Intervention (NVCI) certification (8-hour, non-physical training) within 6 months of hire, and ENPC or TNCC within 6 months of hire.

Estimated Salary

We estimate the annual salary for this position to be around $80,000-$110,000, depending on experience and qualifications.
