Position Overview

Key Responsibilities
  • Assist patients with essential daily living activities, including bathing, grooming, oral hygiene, skin and nail care, shaving, and dressing.
  • Effectively communicate any changes in patient condition to the supervising Registered Nurse (RN) promptly.
  • Document patient complaints and concerns for the RN's review.
  • Maintain accurate and timely records of patient care.
  • Adhere to agency policies, procedures, and all relevant local, state, and federal regulations.
  • Ensure patient safety and the safety of their living environment through careful attention to detail.
  • Report any hazardous conditions in the home environment.
  • Implement effective infection control practices and comply with established protocols.
  • Safely lift and transfer patients as required.
  • Communicate clearly and positively with patients, their families, team members, and other stakeholders.
  • Exhibit a caring, patient-centered approach in all interactions.
  • Keep patients and their families informed about visit schedules and any changes.
  • Participate in in-service training programs to fulfill compliance requirements.
  • Uphold patient confidentiality in accordance with HIPAA regulations.
Qualifications

Essential Requirements
  • Must be a Certified Nursing Assistant (CNA) or possess a current Home Health Aide registration in the state of Indiana.
  • High school diploma or equivalent is required.
  • Valid driver's license and automobile insurance in the state of Indiana are necessary.
  • Ability to provide personal transportation to assignments.
  • Capability to work independently while also supporting a collaborative team environment.
