Key Responsibilities
  • Assist patients with daily living activities, including bathing, grooming, oral hygiene, skin care, and dressing.
  • Promptly communicate any changes in patient condition to the supervising Registered Nurse (RN).
  • Document patient complaints and observations accurately.
  • Maintain compliance with agency policies, as well as local, state, and federal regulations.
  • Ensure a safe environment for patients by paying attention to detail.
  • Report any unsafe conditions observed in the home.
  • Adhere to infection control protocols and practice safe lifting techniques.
  • Foster clear and positive communication with patients, families, and team members.
  • Exhibit a caring demeanor, prioritizing the needs of patients.
  • Keep patients and families informed about visit schedules and any changes.
  • Participate in required training programs to ensure compliance.
  • Uphold patient confidentiality in accordance with HIPAA regulations.
Qualifications

Essential Requirements
  • Must be a Certified Nursing Assistant (CNA) or possess a current Home Health Aide registration in the state of Indiana.
  • High school diploma or equivalent is required.
  • Valid driver's license and automobile insurance in the state of Indiana are necessary.
  • Ability to provide personal transportation to various assignments.
  • Capability to work independently while also supporting a collaborative team environment.
