VNS Health is seeking a skilled Registered Nurse to serve as a Field Supervisor in our Home Health division. As a key member of our clinical team, you will be responsible for providing case management support, developing care plans, and ensuring exceptional clinical outcomes for our patients.
Key Responsibilities- Case Management: Provide comprehensive case management support to patients, including clinical intervention, care plan development, and follow-up support.
- Clinical Leadership: Serve as a clinical leader and mentor to Home Health Aides, ensuring that every patient receives top-quality support and care.
- Field Supervision: Conduct initial and ongoing supervisory field visits to patients' homes to provide assessment, training support, and in-home coordination.
- Documentation: Extensively document every patient visit in the Electronic Medical Record (EMR), ensuring timely and accurate charting for clinical team reference and auditing purposes.
- Current License: Current license to practice as a Registered Nurse in New York State.
- Experience: Minimum of two-year experience as an RN in a home care setting.
- Education: Bachelor's degree in Nursing or related field.
