Job Summary:

We are seeking a skilled Occupational Therapist to join our team at Andwell Health Partners. As an Occupational Therapist, you will play a vital role in enhancing the quality of life of our patients by providing innovative and compassionate care in the comfort of their homes and communities.

About the Role:

This is a full-time opportunity for an Occupational Therapist to work with our team in Southern Maine. The ideal candidate will have a Bachelor's, Master's or Doctorate Degree in Occupational Therapy from an accredited institution and at least one year of clinical experience in Occupational Therapy. Home Health experience is also preferred. The successful candidate will be licensed to practice in the State of Maine and hold a current CPR Card. A reliable and insured automobile and a current Maine driver's license are also required.

Responsibilities:

The Occupational Therapist will be responsible for developing and implementing occupational therapy plans of care that incorporate realistic and measurable outcomes. The successful candidate will provide education and promote client rights and responsibilities for informed decision making in the plan of care. They will also develop, implement, and revise plans of care that incorporate goals consistent with the client's age, condition, and needs. The Occupational Therapist will continually assess the client's needs for occupational interventions and evaluate the effectiveness of the interventions on the occupational visit report. They will refer to internal and external community resources to meet client outcomes when appropriate and initiate diagnostic, preventive, and rehabilitative procedures as necessary to assist in the development and revision of the plan of care. The successful candidate will evaluate and document progress toward outcomes and coordinate discharge plans with clients, families, and other health professionals.
