Job Summary

Critical Connection, inc is seeking a skilled Home Health Registered Nurse (RN) for a 13-week travel nursing contract opportunity in Frederick and Washington County, MD.


Job Description & Requirements

As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. You will work closely with patients and their families to develop and implement individualized care plans, addressing their unique needs and goals.


Responsibilities Include:

  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement personalized care plans to address patients' needs and goals
  • Provide skilled nursing care, including wound care, medication management, and education
  • Collaborate with patients' healthcare teams to ensure seamless care transitions
  • Document patient progress and outcomes in a timely and accurate manner

Requirements
  • Education: Graduate of an accredited nursing program
  • Licensure: Current nursing license, in good standing with the State licensing board
  • Experience: Minimum of one (1) year HH experience
  • Oasis Start of Care: Required
