LPN Home Health Care Specialist

We are seeking a skilled LPN to join our Home Health team in Cleveland. As a Home Health LPN, you will work under the supervision of a Registered Nurse to provide routine care and general observation of clients' health status.
Key Responsibilities- Provide 1:1 care for clients following their plan of care, maintaining their dignity and independence.
- Monitor clients' vital statistics and report abnormal findings to the Case Manager and Physician.
- Administer medications, feedings, oxygen, and ostomy care as needed, per client's plan of care and provided assessments of results.
- Complete routines established by other care professionals, such as PT, OT, and Speech Therapists.
- Ensure infection control policies are maintained at all times, reporting possible communicable diseases or infections and implementing infection control procedures.
- Accurately document nursing actions, communication with family, case manager, and physicians, and document progress and outcomes for established goals.
- Coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are on hand.
- Monitor incidents involving individuals, ensuring incident reports are completed timely and accurately, and informing case managers.
- Ensure all medical equipment is maintained and report faulty equipment to DME for repair.
- LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
- Valid driver's license, acceptable driving record, and proof of car insurance.
- CPR professional certification.
- Experience in administering medications.
- 1+ years of nursing experience preferred.
- Home health care experience preferred.
- Able to lift and carry items weighing up to 20 pounds.
- Able to bend, stoop, push, pull, sit, and walk for periods of time.
- Must be able to communicate both verbally and in writing.
