Job Title: Registered Nurse Clinical Assessment Advisor

At CenterWell Home Health, we are seeking a highly skilled Registered Nurse Clinical Assessment Advisor to join our team. As a key member of our clinical staff, you will be responsible for overseeing the development and completion of treatment plans, evaluating patient needs, and coordinating a team of professionals and clinicians in the home health setting to ensure optimal outcomes.

This role requires a bachelor of science degree in Nursing (BSN) and a minimum of 2 years of clinical home care experience, with at least one year in a clinical management role. You will be responsible for coordinating care team members, ensuring compliance with state and federal regulations, performing OASIS visits, and maintaining effective communication with all stakeholders.

Key Responsibilities:
  • Develop and implement treatment plans that meet the unique needs of each patient
  • Evaluate patient needs and coordinate care with other healthcare professionals
  • Ensure compliance with state and federal regulations and guidelines
  • Perform OASIS visits and document patient information
  • Maintain effective communication with care team members and stakeholders
Requirements:
  • Bachelor of Science degree in Nursing (BSN) preferred
  • At least 2 years clinical home care experience with minimum one year experience in a clinical management role
  • Current CPR certification
  • Excellent verbal and written communication skills
  • Knowledge of state and federal home health agency regulations and compliance standards and regulations
  • Knowledge of clinical policies and procedures and ability to implement
  • Knowledge of clinical structure of PDGM
  • Must read, write and speak fluent English
