RN Clinical Assessment Advisor, Home Health

We are seeking a skilled RN Clinical Assessment Advisor to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in delivering high-quality patient care and ensuring the best possible outcomes for our patients.
Key Responsibilities- Conduct comprehensive assessments of patients' needs and develop personalized care plans to meet their unique requirements.
- Collaborate with healthcare professionals, including physicians and other licensed clinicians, to ensure seamless care coordination and optimal patient outcomes.
- Coordinate the delivery of care services, including home health aide visits and other interventions, to ensure patients receive the support they need to recover and thrive.
- Monitor patient progress, identify areas for improvement, and make adjustments to care plans as needed to ensure the best possible outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure that all parties are informed and engaged in the care process.
- Bachelor of Science degree in Nursing (BSN) preferred.
- At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role.
- Current CPR certification.
- Excellent verbal and written communication skills.
- Excellent interpersonal skills.
- Knowledge of state and federal home health agency regulations and compliance standards.
- Knowledge of clinical policies and procedures and ability to implement.
- Knowledge of clinical structure of PDGM.
- Must read, write, and speak fluent English.
- Must have good and regular attendance.
- Approximate percent of time required to travel: 60%.
