Patient Advocate RN

We are seeking a dedicated Patient Advocate RN to join our team at EMrecruits in Fayetteville, North Carolina. As a Patient Advocate, you will be responsible for ensuring patients receive comprehensive, coordinated care that meets their unique needs and preferences.
Estimated Salary: $70,000 - $85,000 per year
Responsibilities- Develop and implement individualized care plans with patients and families
- Coordinate with healthcare providers, community agencies, and other stakeholders to ensure seamless care transitions
- Maintain accurate records and communicate effectively with patients and families
- Act as a liaison between patients, providers, and community agencies to advocate for patients' needs and preferences
- Educate patients on disease management and medication adherence
- Support patients in addressing concerns and accessing community resources
- Degree in Nursing (RN or LPN) or RMA
- Experience in direct patient care or managed care preferred
- Valid driver's license and personal vehicle required for travel
- HEDIS Quality Measures experience and proficiency in Microsoft Office
- Excellent communication and customer service skills
- Ability to work independently and collaborate with teams
