Key Responsibilities:
1. Patient Evaluation and Care:
- Perform comprehensive evaluations and create personalized care strategies tailored to each patient's age and medical condition.
- Observe and assess patient progress, making necessary modifications to care strategies as needed.
2. Team Collaboration:
- Oversee a patient caseload, working collaboratively with a multidisciplinary team to guarantee holistic care.
- Supervise home health aides and coordinate nursing services with LPNs, ensuring compliance with established protocols.
3. Training and Development:
- Deliver practical clinical training and mentorship to new nursing staff, promoting their professional development and proficiency.
- Regularly evaluate and review the performance of new clinicians, providing constructive feedback and support.
4. Documentation and Communication:
- Ensure accurate and timely documentation of patient care activities, adhering to agency policies.
- Communicate effectively with patients, families, and healthcare professionals to foster coordinated care.
5. Quality Enhancement:
- Engage in ongoing quality improvement initiatives, contributing to the overall success of the agency's programs.
- Uphold the agency's mission, vision, and values in all interactions and care delivery.
Qualifications:
- Graduate of an accredited nursing program (AD, Diploma, or BSN).
- Current nursing license in the relevant state and CPR certification.
- Minimum of one year of RN experience, with additional experience in home health preferred.
Physical and Mental Requirements:
- Ability to perform physical tasks associated with patient care, including lifting and transferring patients.
- Strong assessment and critical thinking skills to formulate effective care plans.
