Position Overview:
As a vital member of our healthcare team, the Advanced Practice Provider plays a crucial role in delivering high-quality care to our senior patients. This position is part of Humana's commitment to providing exceptional value-based care through our network of clinics.

Key Responsibilities:
1. **Patient Evaluation and Treatment:**
- Conduct thorough assessments and develop treatment plans for patients, ensuring adherence to established standards of care.
- Monitor patient progress and adjust care plans as necessary, utilizing available data and medical records.

2. **Collaboration and Coordination:**
- Work closely with a multidisciplinary team to coordinate patient services, including specialty referrals and home health care.
- Participate actively in daily team meetings to foster a collaborative environment.

3. **Documentation and Compliance:**
- Maintain accurate and timely medical records, collaborating with coding specialists to ensure optimal coding practices.
- Uphold compliance with all licensing and accreditation requirements.

Qualifications:
- A minimum of two to five years of relevant experience is preferred.
- Successful completion of a nurse practitioner program with board certification.
- Active and unrestricted nurse practitioner license in the state of practice.
- Strong communication skills, both written and verbal, with a focus on teamwork and integrated care.
- Basic computer proficiency, including experience with electronic medical records.

Preferred Qualifications:
- Active DEA license and Medicare Provider Number.
- Experience in managing a Medicare Advantage patient panel.
- Knowledge of Medicare guidelines and HEDIS quality indicators.
- Bilingual skills, particularly in Spanish, are advantageous.
